It doesn’t matter if you call it an ‘Exchange Program’ or a simple ‘recall’ but the crux of the matter is that iPhone users face a risk of electric shocks by using the USB Power Adapters. The power adapter’s metal prongs can break off and remain in a power outlet thus creating a risk of electric shock. Apple acknowledges it and all users who have got this bundled or bought as a standalone accessory from affected countries (list below) can opt in for the program.

Users having a green dot on the bottom of the adapter have the newly designed adapters and are safe to use.

Affected Countries:

US
Japan
Canada
Mexico
Colombia
Ecuador
El Salvador
Guatemala
Honduras
Peru

Latin American countries where adapters were sold only as a standalone accessory:

Argentina
Chile
Costa Rica
Panama

Users can either order the new adapter online or head over the nearest retail store from October 10.
